1. Go to Community Foundation (for the Fox Valley Region)
2. Under the Donation Information section:
   • Enter Donation amount
   • In the Designation field: Select “OTHER” from the drop-down list
   • Type in RHI or Rural Health Initiative in the text box
3. Complete Additional, Billing, and Payment Information sections.
4. Click “Donate Now”
