Meet our team:

 

We are excited to provide a higher level service that is convenient to our rural individuals. People who have barriers to care because have a hard time coming in due to transportation issues, working long hours or odd times, or who don’t have a doctor and simply don’t know who or where to go for care. We want to help you get the care you need when you need it.

Basic education discussion at no cost such as:

    • Health promotion – disease prevention
    • Assess for Health Maintenance needs- discuss testing or vaccines needed
    • Medication Reconciliation- helping you understand your medications
    • Chronic Disease Evaluation

 

Arrange to meet with one of our Nurse Practitioners

All appointments with our Nurse Practitioners, whether on site or virtual, are set up through each county’s Outreach Healt Specialist.  Once you have spoken with your OHS, they will get an appointment set up with the Nurse Practitioner.  Please click the button below to first call your county’s Outreach Health Specialist to get the process started!

Evaluation and treatment of each patient is individualized and is based on best practices.
This is not an exclusive list, treatments and procedures are at the discretion of the provider.

Services Not Provided:

    • Sports/Camp Physicals

    • Workman’s comp injuries

    • DOT physicals

    • Obstetric services/procedures

    • Orthopedic services such as casting

    • Allergy injections

    • Travel medicine

    • Prescribing of controlled substances

    • Complex psychological treatment, such as schizophrenia

    • Cardiology services such as EKG’s

    • Pulmonary Services (sleep apnea testing etc.)

    • Gynecological procedures/PAP

    • Uncontrolled Chronic disease (refer to PCP for management)

    • Well Child, Annual physicals or pre-operative physicals (refer to PCP)

The costs associated with lab work and diagnostics related to the management of health conditions are billable charges.

The Virtual or in-home visits are an extension of the Primary Care Physician (PCP), not a replacement. Establishment and/or maintenance of a relationship with a PCP for preventative care and chronic disease and medication management will facilitated.
